Job Summary: Oversee and develop culturally competent, comprehensive, and high-quality services for our Ryan White HIAP and Benefits Counseling programs. Maintain program standards for regulatory compliance and consumer satisfaction. Ensure that program goals and objectives are achieved. Responsible for staff supervision, quality assurance and coordination of services.
Major Activities:
Incorporate best practices regarding confidentiality into all job duties and communications in accordance with HIPAA, Article 27-F, Alliance for Positive Health policies and procedures and other applicable regulations. Observe and abide by HIPAA and the HIV Confidentiality Law.
Conduct eligibility screens to ensure potential clients are appropriate for HIAP and Benefits Counseling.
Conduct intakes and required assessment at time of intake, assure that all intake documents are uploaded to the Electronic Health Record (EHR) before assigning the client to a staff member.
Assure best practices in delivery of program services. Develop and administer strategies to increase engagement in agency services and ensure positive health outcomes for target populations. Adapt and tailor as applicable to designated target population(s). Ensure that the Transtheoretical Model of Behavior Change is incorporated, as applicable, into all aspects of programming.
Assists in onboarding and training processes of new staff.
Actively participate in agency wide leadership meetings to ensure effective internal communication and procedures.
Conduct quality assurance review and provide feedback to staff to ensure deliverables meet funder and agency standards. Review reporting forms and client files completed by staff to ensure documentation is thorough, complete and in compliance with program standards.
Provide supervision on a regular basis for designated staff. Responsibilities include staff development, training, timely performance appraisals, and managing performance as needed.
Complete Certified Application Counselor (CAC) training through the New York State of Health (NYSOH) marketplace and Workforce Incentive Practitioner Credentialing (WIP-C) through Cornell University.
Coordinate and/or submit services documentation, client health indicator, contact, history and other data requirements and outcomes reporting in accordance with program standards. Ensure effective communication of client health information among internal and external interdisciplinary teams.
Perform other duties as assigned.
Qualifications:
- Associate's degree in human services or related field.
- Four years of experience in human services field; two of which must be in a supervisory capacity.
- 1-2 years of experience working in the field of HIV/AIDS or other chronic illnesses.
- Experience working with electronic medical records and/or health information exchanges (e.g. HIXNY) preferred.
- Must have a valid, unrestricted driver's license and own vehicle for frequent travel.
- Spanish language proficiency a plus.

- Excellent time management and organizational skills; ability to manage multiple tasks and priorities with attention to detail.
- Knowledge of best practices as applicable to assigned programs and services.
- Ability to assess and evaluate community trends and recommend adaptation to service delivery. Receptiveness to community feedback and input.
- Ability to build and support team rapport.
- Ability to maintain professional boundaries within agency and grant guidelines, as well as with clients and external service providers.
- Bilingual a plus.
Employment Classification: Exempt, Fulltime; some nights and weekends may be required.
