Overview

Unified Community Services is seeking to hire two full-time Support and Service Coordinator positions serving Grant and Iowa Counties in our Birth to Three, Children's Long-Term Support and Children's Community Options team. This role helps families in our community navigate services and supports for children with disabilities. They act as a point of contact, helping families access resources, develop plans and coordinate care and are a family's partner in accessing and managing the support they need. These coordinators also help families access the early intervention system, identify appropriate supports and services, and navigate our programs.

This position requires a Bachelor's Degree in social work and/or human service-related field and a minimum of 2-years' experience working with families and children with special needs.

Social Worker Certification required or the certification process has been started at hire and completed within two years of hire required to maintain position. Position requires a valid driver's license, reliable transportation and completion of caregiver background check.
