We are seeking an SMI Care Manager to join our team at Independent Living Systems (ILS). ILS, along with its affiliated health plans known as Florida Community Care and Florida Complete Care, is committed to promoting a higher quality of life and maximizing independence for all vulnerable populations.

About the Role:

The SMI Care Manager plays a critical role in coordinating and managing care for individuals with Serious Mental Illness (SMI), ensuring they receive comprehensive, person-centered support tailored to their unique needs. This position involves collaborating with multidisciplinary teams, including healthcare providers, social workers, and community resources, to develop and implement effective care plans that promote recovery and improve quality of life. The Care Manager acts as a primary point of contact for members and their families, facilitating communication and advocating for necessary services and interventions. By monitoring member progress and adjusting care strategies accordingly, the role aims to reduce hospitalizations and enhance overall health outcomes. Ultimately, the SMI Care Manager contributes to building a supportive environment that empowers members to achieve stability and independence within their communities.

Minimum Qualifications:

  • Bachelor's degree in Social Work, Nursing, Psychology, or a related health care field.
  • Current and valid professional licensure or certification relevant to care management (e.g., RN, LPN or MSW).
  • At least 2 years of experience working with individuals diagnosed with Serious Mental Illness (SMI).
  • Knowledge of mental health care systems, community resources, and relevant regulatory requirements.
  • Ability to maintain confidentiality and adhere to ethical standards in member care.
  • Relevant experience may substitute for the educational requirement on a year-for-year basis.

Preferred Qualifications:

  • Master's degree in Social Work (MSW), Nursing (MSN), or a related clinical discipline.
  • Certification in Case Management or Mental Health Care Coordination.
  • Experience with electronic health records (EHR) and care management software.
  • Familiarity with trauma-informed care practices and recovery-oriented approaches.
  • Bilingual abilities or experience working with diverse cultural populations.

Responsibilities:

  • Conduct comprehensive assessments of members' mental health status, social needs, and functional abilities to inform individualized care planning.
  • Develop, implement, and regularly update personalized care plans in collaboration with members, families, and healthcare professionals.
  • Coordinate access to medical, psychiatric, and social services, ensuring timely referrals and follow-up.
  • Monitor member progress through regular contact, adjusting care plans as needed to address changing needs or challenges.
  • Advocate for members' rights and needs within healthcare systems and community resources to promote equitable access to services.
