Responsibilities

  • Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support.
  • Monitor residents' health by taking vital signs such as blood pressure, pulse, and temperature; observe for any changes in condition or behavior.
  • Support residents with medication reminder following care plans and HIPAA guidelines to ensure privacy and safety.
  • Prepare nutritious meals and assist with meal planning in accordance with dietary restrictions and preferences.
  • Maintain a clean and safe environment by performing light cleaning duties and ensuring proper sanitation protocols are followed.
  • Document care activities accurately in compliance with facility policies and state regulations; update care plans regularly.
  • Engage residents in social activities to promote mental stimulation and emotional well-being, especially those with dementia or Alzheimer's disease.
  • Collaborate with nursing staff, social workers, and family members to coordinate comprehensive care tailored to each resident's needs.
  • Follow all safety procedures including CPR, first aid, and emergency response protocols to ensure resident safety at all times.
  • Support residents' participation in community outings or activities that enhance quality of life.

Experience

  • Prior experience working in a home health care or is highly preferred.
  • Demonstrated knowledge of patient observation techniques and ability to monitor vital signs accurately.
  • Familiarity with HIPAA regulations and maintaining confidentiality of patient information is essential.
  • Hands-on experience with patient care procedures including medication reminder
  • Ability to work effectively with diverse populations including seniors, individuals with disabilities, or those requiring hospice or palliative care.
  • Knowledge of infection control practices and basic cleaning procedures to uphold hygiene standards.
  • Excellent communication skills for collaborating with team members and engaging residents compassionately.
