We are seeking a Physical Therapist to join our team to work in a unique, collaborative advanced primary care team model. As a provider, you are focused on building strong relationships with your patients to address their needs beyond their symptoms.
The ideal candidate will have ample experience to not only evaluate and manage patients but to contribute to a larger medical team in service to shared customers' wellbeing initiatives. Candidates should thrive on being a primary care provider for musculoskeletal conditions within a larger medical team. Candidates will need to excel at communication and crave collaboration using shared decision making with both patients and the medical team.

Here's how you will make an impact
Evaluates physical and functional status, as well as cognitive, psychological, and perceptual ability to determine the patient's capabilities to function (safely) Documents all evaluations, treatment, and coordination of care contacts via appropriate facility policies Demonstrates an ability to treat a wide range of diagnoses and be knowledgeable in current therapy treatments, goals, and contraindications for treatment Provides education and counseling to patient/family/clients/providers regarding treatments and goals, potential safety risks and/or other issues within scope of practice Independently manages own case load to maximize effectiveness of overall care plan Complies with all applicable Federal, State, and local laws, regulations, and requirements, as well as facility/agency specific policies and procedures. Supports special projects and other designated activities as assigned in support of TAI's mission and core values Works collaboratively with care team members to include but not limited to, MD, PA, RN, Behavioral Psychologist, Health Coach, Nutritionist, Care Management, etc. Responsible to managing and driving MSK care within Care Team
What you will need to bring to the table
Active Physical Therapist license 4+ years of clinical experience Commitment to top quality patient care. Eagerness to develop your skills and support your team members as they develop their skills. OCS and/or NAIOMT certification preferred
Physical Therapist base compensation is $98,000-$108,000+ depending on experience and fit. Relocation/sign-on bonus available
