Reports To: Director of Clinical Services
Job Description SummaryThe Clinical Manager is responsible to promote MAHADD'S organizational directives for clinical services. The Clinical Manager establishes, implements, and evaluates goals and objectives for home health services that meet and promote the standards of quality and contribute to the total organization and philosophy. Clinical manager is expected to ensure patient care coordination is managed effectively day-to-day with GRIT and integrity. Assures quality and safe delivery of home health services are provided throughout the organization.
Essential Job Functions/Responsibilities- Reviews patient information related to case, including disciplines required, to determine home care needs. Assigns appropriate clinicians to case to ensure coverage for all clients of MAHADD Home Health Care.
- Reviews and evaluates cases by reviewing the services provided by clinicians with record review, discussion and mentorship, and guides clinicians to promote effective performance.
- Is available at all times during operating hours to assist clinicians as appropriate. Attends case conference meetings, and Interdisciplinary Team meetings, with organization personnel to facilitate coordination of care.
- Conducts quarterly record reviews and communicates findings and recommendations to Clinical Director and appropriate organization personnel.
- Assists in the screening and interviewing process of new organization personnel and makes recommendations for employment of individuals. Assists in the orientation of new organization personnel. Hires, evaluate (annually or more frequently), and terminate organizational personnel.
- Assists Clinical Director in the planning, implementation and evaluation of in-services and continuing education programs.
- Assists in the formulation, revision, implementation, and evaluation of policies and procedures, as well as strategic goals and objectives.
- Complies with accepted professional standards and principles. Promotes customer service orientation to all organization personnel.
- Participates in public relation and community activities that promote MAHADD'S role as an effective member of the health care delivery system.
Performs other duties and activities as delegated by the Clinical Director.
Participates meaningfully in all team meetings aimed at continuity of care, or care conferences including Interdisciplanary Team Meetings, and Case Conferences.
- Participates in the Intake process, and coordination of new Home Health referrals.
Additional Duties
- Participates in on-call duties as defined by the on-call policy and patient census.
- Ensures equipment, necessary supplies, and services are available prior to making home visits, or arranging visits on behalf of field staff.
- Observes and interprets operational indicators to detect cnsus changes, which impact staffing levels, revenue and expenses, and collaborates proactively to project needs.
- Assures compliance with all local, state adn federal laws regarding licensure and certification of organization personnel and , maintains compliance to the ACHC Home Care standards.
- In the absence of the Director of Clinical Services/Administrator, the Clinical Manager will become the acting Director of Clinical Services/Administrator and will be vested with the authority to act in behalf of the Director of Clinical Services/Administrator.

- Registered Nurse with current licensure to practice professional nursing in the state of MN or ability to obtain prior to hire, or Certified Occupational Therapist, or Physical Therapist.
- Graduate of accredited school of nursing. BSN degree is preferred.
- Two (2) years management or supervisory experience in home care.
- Complies with accepted professional standards and practice.
- Must have Current BLS certification or obtain prior to hire.
Demonstrates excellent observation, verbal and written communication skills.
Must have GRIT and Integrity.
- Prolonged or considerable walking or standing. Able to lift, position or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling or crouching. Visual acuity and hearing to perform required nursing skills.
- Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.
