Patient Care Coordinator

Position Summary:
The Customer Service Representative (CSR) is a field-based role responsible for delivering patient monitoring equipment, admitting patients into our program, and ensuring accurate and timely completion of all required documentation. This position involves visiting patients in their homes, providing program information, collecting signed paperwork, and setting up equipment. The CSR also performs follow-up visits to deliver additional supplies, conduct wellness checks, and ensure patient satisfaction. A pre-planned schedule is provided daily; strong time management and organizational skills are essential to success.
Key Responsibilities:
Patient Admissions & Onboarding
- Deliver patient monitoring equipment to homes in a timely and professional manner.
- Provide a clear and friendly program overview and explain all paperwork to ensure patient understanding and consent.
- Accurately collect and complete required forms and upload them into the company's platform before the end of the shift.
- Set up equipment as needed and ensure the patient is comfortable using it.
Follow-Up Visits & Wellness Checks
- Deliver additional supplies to patients as scheduled.
- Conduct wellness checks during visits, documenting observations and escalating concerns to the appropriate department.
- Maintain a professional and compassionate approach during all patient interactions.
Route & Time Management
- Follow the schedule and route plan to maximize efficiency.
- Communicate with supervisors regarding delays, changes, or issues encountered in the field.
- Maintain accurate records of all visits, deliveries, and interactions.
Qualifications:
- High school diploma or equivalent required.
- Valid driver's license with a clean driving record.
- Strong interpersonal and communication skills.
- Basic technology skills for using a tablet, smartphone, and uploading documentation.
- Excellent time management and ability to work independently.
- Previous experience in customer service, healthcare, or delivery roles preferred.
- MULTILINGUAL - SPANISH PREFFERED

- Comfort with light equipment handling and setup.
Work Environment:
This is a field-based role requiring full-day travel to patient homes. Work is performed primarily in patient residences and in a vehicle while traveling between appointments.
