Program Manager II, Social Drivers of Health

This position leads the development, implementation, project management, and evaluation of Social Drivers of Health (SDOH) and Health-Related Social Needs (HRSN) initiatives. The role supports a broad portfolio of work that advances organizational capabilities to address SDOH and HRSN. Key areas of focus include: expanding and optimizing the use of social needs platforms, standardizing documentation and data capture of member HRSN across systems; identifying financing non-benefit-based services; and developing data infrastructure to support shared analytics, continuous improvement, and strategic decision-making. This position collaborates cross-functionally with internal teams and external partners to ensure alignment with regulatory requirements, accreditation standards, and member-centered design principles.
To be successful in this role, you:
- Have a minimum of three (3) years professional experience in a direct health care delivery, managed care, public health, healthcare consulting, or relevant sector experience.
- Have a minimum of three (3) years leading programs or projects.
- A Bachelor's Degree or Master's Degree is considered.
Essential functions and Roles and Responsibilities:
- Develop and implement SDOH and HRSN initiatives. Provide regular communication on progress to timeline and goals.
- Evaluate effectiveness of initiatives and interventions and present recommendations to leadership.
- Manage cross-functional teams with internal and external stakeholders that advance the SDOH and HRSN initiatives, including collaboration with leaders on work plan development and tracking. Provide regular communication on progress to timeline and goals.
- Conducts research and analysis on best practices to inform initiative interventions and sustainable funding model designs.
- Participate in committees, work groups and advisory groups and be responsible for specific tasks that advance the work. Develop meeting agendas and presentations in collaboration with SDOH Director and other focus area leads for relevant meetings.
- Serve as point of contact and subject matter expert for internal and external contacts related to SDOH and HRSN initiatives.
- Prepare and present presentations, reports, applications, and/or other documents associated with moving work plans forward.
- Represent CHPW with external stakeholders.

Compensation and Benefits:
The position is FLSA Exempt and is not eligible for overtime. Based on market data, this position grade is 66 and has a 10% annual incentive target based on company, department, and individual performance goals. Salary determined at offer will be based on labor market data and a candidate's years of relevant work experience and skills relevant to the position.
