Resolve Harmony is a caring and compassionate skilled nursing facility dedicated to providing exceptional long-term care and short-term rehabilitation services. We are seeking a knowledgeable, resident-focused Social Services Director to join our leadership team and help ensure the emotional, social, and psychosocial well-being of our residents.
Position Summary
The Social Services Director is responsible for planning, developing, organizing, implementing, and evaluating the overall social services program within the facility. This role ensures that each resident receives the support they need to achieve their highest practicable level of well-being while maintaining regulatory compliance.
Key Responsibilities
- Assess residents' psychosocial needs and develop individualized care plans.
- Participate in interdisciplinary care team meetings, MDS processes, and discharge planning.
- Provide counseling, crisis intervention, and family support as needed.
- Coordinate community resources and services for residents and families.
- Manage admission and discharge processes, ensuring smooth transitions and regulatory compliance.
- Advocate for residents' rights and serve as a liaison between residents, families, and facility staff.
- Maintain accurate and timely documentation per state, federal, and facility standards.
- Oversee and train Social Services staff (if applicable).
- Support Quality Assurance/Performance Improvement (QA/PI) initiatives.
Qualifications
- Bachelor's degree in Social Work (BSW) or related human services field required; Master's degree (MSW) preferred.
- Licensed Social Worker (LSW, LCSW, LMSW, or equivalent) as required by state law.
- Minimum 1–2 years of social work experience in a long-term care or healthcare setting (SNF preferred).
- Strong knowledge of state/federal regulations for nursing facilities, including CMS guidelines.
- Excellent communication, documentation, and organizational skills.
- Compassionate, resident-centered approach.
