Job Responsibilities:

  • Direct all daily patient social service operations including providing direct oversight of the establishment and implementation of social service policies.
  • Ensures compliance with all state, federal, and CHAP social service regulatory requirements.
  • Directs the implementation of improved work methods and procedures to ensure social services are delivered in accordance with policy.
  • Ensures seamless home care service delivery by providing direct oversight of coordination of care with other clinical service providers.
  • Assists the Executive Director/Administrator in the preparation of an annual budget for the social service department and monitors allocation of resources according to budgetary limitations.
  • Recruits, selects, orients, and directly manages members of the social service teams.
  • Supervises the assessment, planning implementation, and evaluation of social service clinical care by members of the social work team.
  • Assures proper documentation of social services in clinical records in compliance with state, federal, and CHAP standards.
  • Provides leadership in strategic planning including identifying opportunities for additional or improved services to address unmet customer needs.
  • Maintains comprehensive working knowledge of the field of social work and shares information with appropriate organization personnel.
  • Maintains comprehensive working knowledge of Hospice Promise LLC contractual relationships and ensures that social services are delivered according to contract provisions.
  • Maintains comprehensive working knowledge of community resources and assist referral sources in accessing community resources should service not be provided by Hospice Promise LLC.
  • Plans and implements in-service and continuing education programs to meet the development needs of social service personnel.
  • Actively participates in quality assessment performance improvement teams and activities.
  • Must be able to load, unload, move and place patient equipment of 50 pounds during shift.
  • Must be able to load and unload medical supplies into car and into patient home.
  • Must be able to bend, twist, reach and pull during shift.
  • Must be able to operate a computer during shift.
  • Must be able to drive during shift.
  • Must be able to exchange accurate information with patients and their families.
  • Must be able to be stationary and in a sitting position during shift.
  • Must be able to walk throughout the office and patient homes during shift.
  • Must be able to complete tasks, including learning, remembering, focusing, categorizing, and integrating information for decision making, problem-solving, and comprehending.

Position Qualifications

  • Master's degree in social work. Meets personnel qualifications stated in § b)(3).
  • At least three (3) years management experience preferably in home care operations.
  • Demonstrates an ability to supervise and direct professional and administrative personnel.
  • Ability to deal tactfully with customers and the community.
  • Knowledge of corporate business management, governmental regulations and CHAP standards.
  • Demonstrates good communications and public relations skills.
  • Demonstrates autonomy, organization, assertiveness, and flexibility and cooperation in performing job responsibilities.
  • Personal car for travel and valid driver's license.
  • Carry personal auto liability insurance coverage.
