Duties

  • Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
  • Monitor patients' health status by observing vital signs and overall condition; report changes promptly to healthcare professionals
  • Provide medication reminders and support medication administration in accordance with care plans and HIPAA regulations
  • Prepare nutritious meals tailored to individual dietary needs and assist with feeding as necessary
  • Use specialized equipment such as Hoyer lifts or resident lifts to ensure safe transfers and mobility assistance
  • Maintain a clean and safe environment by performing light housekeeping tasks including cleaning, laundry, and organizing personal spaces
  • Document care activities accurately in compliance with privacy standards such as HIPAA; update care plans regularly
  • Support clients with cognitive challenges such as dementia or Alzheimer's disease through engagement and memory care techniques
  • Assist with transportation to appointments or community activities when needed
  • Collaborate with family members, healthcare providers, and social workers to ensure comprehensive care delivery
  • Follow all safety protocols including first aid procedures and CPR when necessary; be prepared for emergency situations

Requirements

  • Prior experience in home health, assisted living, nursing homes, group homes, or hospice care is highly preferred
  • Knowledge of patient observation techniques and monitoring vital signs accurately
  • Familiarity with working with individuals who have developmental disabilities, autism spectrum disorder, or dementia-related conditions
  • Certification in CPR and first aid required; additional certifications such as HIPAA training are a plus
  • Ability to lift heavy objects safely and perform physically demanding tasks using equipment like Hoyer lifts or resident lifts
  • Strong communication skills to effectively interact with clients, families, and healthcare teams
  • Compassionate attitude with a genuine desire to provide excellent patient-centered care
  • Valid driver's license may be required for transportation duties; clean background check necessary
