This role is a hybrid model in Boston, Massachusetts.
Community Wellness Coordinator Responsibilities
- Build trusting relationships with patients through in-person outreach, needs assessments, goal setting, and ongoing progress monitoring.
- Coordinate community, home-based, and clinical support by connecting patients with social services, educational resources, and care team partners.
- Provide culturally aligned health education, motivational interviewing, and resource navigation across diverse community settings.
- Document assessments, care plans, and patient updates in electronic systems while preparing reports and contributing to case reviews.
- Support patients in overcoming barriers by assisting with applications, appointments, prescriptions, and warm handoffs to community programs.
Community Wellness Coordinator Qualifications
- HS diploma with community experience or Bachelor's degree, plus 2+ years in healthcare, public health, or community-based work.
- Strong interpersonal, communication, and relationship-building skills with the ability to engage diverse, multicultural populations.
- Basic knowledge of healthcare systems, community resources, and social determinants of health.
- Organizational excellence with the ability to multi-task, work independently, and thrive within interdisciplinary teams.
- Valid driver's license, reliable transportation, and proficiency in Microsoft Office; multilingual abilities highly preferred.
