Job Summary

Duties

  • Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using tools like Hoyer lifts when necessary
  • Monitor and document patient health status by taking vital signs such as blood pressure, pulse, temperature, and respiration rates using Electronic Health Record (EHR) systems
  • Support medication administration following prescribed protocols while maintaining strict adherence to HIPAA regulations to ensure patient privacy and confidentiality
  • Provide specialized care for patients with dementia or Alzheimer's disease through memory care techniques and tailored activities
  • Assist with medical procedures such as catheter care, wound dressing changes, and hospice support as directed by healthcare professionals
  • Prepare nutritious meals aligned with dietary restrictions and assist with cooking and meal planning to promote health and comfort
  • Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment to support overall patient well-being

Requirements

  • Proven experience in home health care or long-term care environments such as nursing homes or assisted living facilities
  • Strong knowledge of medical terminology, patient observation techniques, and medical documentation practices
  • Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications like medication administration or hospice care are a plus
  • Experience working with people with developmental disabilities or disabilities in general is highly valued
  • Ability to operate EMR (Electronic Medical Records) systems efficiently for accurate recordkeeping
  • Familiarity with dementia care strategies, Alzheimer's care, memory care techniques, and working within HIPAA guidelines
  • Physical ability to assist with patient transfers using Hoyer lifts or similar equipment; capable of standing, walking, bending, and lifting as needed
  • Compassionate attitude combined with excellent communication skills to work effectively with patients, families, and healthcare teams
