About Us: Western Reserve Area Agency on Aging (WRAAA)
is a private non-profit corporation, organized and designated by the State of Ohio to be the planning, coordinating and administrative agency for federal and state aging programs in Cuyahoga, Geauga, Lake, Lorain, and Medina Counties. It is one of twelve Area Agencies on Aging (AAAs) in the state organized with local service provider organizations and the Ohio Department of Aging (ODA) to form the state's public aging services network. The network collaborates to create opportunities for older Ohioans to receive needed home and community services and support; and to age successfully in their own homes and communities.
Duration:
This is an non-paid internship for the duration of the schools need.
Overview:
This internship is designed for students eager to make an impact on social determinants of health by interacting directly with clients through both telephonic and in-person assessments. Whether you're studying Social Work, Public Health, Human Services, or a related field, you will develop critical skills in assessing client needs, connecting them with community resources, and promoting holistic health and independence.
Key Responsibilities:
- Conduct comprehensive telephonic and in-person evaluations to identify the social determinants affecting clients' well-being.
- Utilize excellent questioning and listening techniques to grasp clients' social, economic, and environmental contexts.
- Collaborate with interdisciplinary teams to craft personalized care plans addressing barriers related to social determinants.
- Facilitate and coordinate access to community services, such as housing assistance, transportation, food programs, and mental health support.
- Maintain precise records of all assessments, interventions, and outcomes in accordance with regulatory and organizational standards.
- Offer ongoing support and follow-up to ensure clients can access the recommended resources and services.
- Keep up to date with changes in social services, community resources, and healthcare policies to provide current information and support to clients.
- Engage with clients, healthcare providers, and community organizations to build collaborative relationships.
- Participate in team meetings and training sessions to enhance your skills in care coordination and client engagement.
Qualifications:
- Enrollment in a Bachelor's or Master's program in Social Work, Public Health, Human Services, or a related field.
- Ability to manage multiple tasks effectively and efficiently.
- Exceptional interpersonal skills for interacting with clients, families, and community partners.
- A focus on promoting client independence and well-being through a compassionate approach.
- Skill in conducting assessments and documenting findings accurately.
- Basic proficiency in Microsoft Office (Word, Excel, Teams) and digital communication tools.
- Strong dedication to understanding and addressing issues related to social determinants of health and promoting health equity.
