Overview

We are seeking a dedicated and compassionate Full-Time Team Lead RN for our Home Health Care. This role involves providing high-quality nursing care to patients in their homes, ensuring their comfort and well-being. The ideal candidate will possess a strong clinical background, excellent communication skills, and a passion for patient-centered care. Experience in various medical settings is preferred, as the role requires adaptability and proficiency in diverse nursing practices.

Duties

  • Conduct comprehensive patient assessments and develop individualized care plans.
  • Administer medications and treatments as prescribed, including IV infusion and injections.
  • Provide wound care, catheter care, and assist with activities of daily living.
  • Monitor vital signs and report any changes in patient conditions to the healthcare team.
  • Educate patients and families on health management, medication administration, and self-care techniques.
  • Collaborate with interdisciplinary teams to ensure coordinated patient care.
  • Maintain accurate medical documentation in compliance with HIPAA regulations.
  • Utilize electronic medical record (EMR) systems for documentation and patient tracking.
  • Perform triage assessments for urgent situations and coordinate emergency interventions when necessary.
  • Support patients with chronic conditions such as diabetes, cardiac issues, or respiratory diseases.

Experience

Candidates should have a robust background in nursing with experience in home health or similar settings. Preferred qualifications include:

  • Proficiency with EMR systems
  • Experience in critical care environments
  • Familiarity with various nursing specialties including geriatrics, pediatrics, hospice & palliative medicine, and behavioral health.
  • Knowledge of medical terminology, ICD coding (ICD-9 & ICD-10), and case management principles.
  • Previous experience working with individuals with developmental disabilities or memory care needs is advantageous.

Pay: $ $47.54 per hour
