Program Manager Social Clinical

GENERAL SUMMARY:
Provides management and administrative activities that support strategic, regulatory, business and program development pursuant to achieving positive health outcomes and meeting system goals for equitable health care outcomes, especially those relevant to addressing social determinant of health.
The Program Manager will be responsible for operationalizing changes to ensure cost, quality, and other key performance indicators are achieved for assigned projects. Supporting the development and management of multi-year scale and integration of evidence-based SDOH screening modalities, electronic integration of community information exchanges, and novel, innovative and relevant programs/interventions that optimize clinical health outcomes, diminish costs, drive value, and improve access to, and quality of healthcare. The Program Manager will closely monitor the growth of social and/or community health programs, processes, regulatory changes, and track the key performance indicators and patient health outcomes as they are determined. The Program Manager will provide regular updates to senior management on assigned program outcomes and opportunities for continued program enrollment growth in identified clinics for addressing health disparities.
Liaising between CIN, CHEW, and Primary Care, and supporting the system leadership councils, the Program Manager will help build awareness and facilitate the coordination of collaborative opportunities that leverage interdepartmental expertise in community health, social determinants, value-based care, clinical operations, quality, and evaluation. Guided by leadership, the Program Manager will work to ensure that efforts related to the organization's strategic and operating plans are supported and reported in a fashion that demonstrate impact regarding better care outcomes and improvements in health of the community, while also identifying efficiencies to mitigate cost. Additionally, they will focus on translating efforts into sustainable practices through existing policy revisions or the development of new policies.
PRINCIPLE DUTIES AND RESPONSIBILITIES:
- Develops growth strategies that promote program sustainability in alignment with department goals including, but not limited to, the creation of corresponding and detailed business plans that include market demand assessment, return on investment modeling, and feasibility analysis, to ensure continued replication and scale of impactful initiatives.
- Coordinates and monitors patient enrollment and programmatic growth, ensuring adherence to established timelines and deliverables.
- Implements effective project management methodologies and tools to track progress and evaluate outcomes and impact.
- Effectively interfaces with clinic administrators, program leaders and community partners and other stakeholders to maximize impact, ensure alignment and foster effective communication.
- Monitors and ensures compliance with administrative and regulatory policies and procedures influencing and informing tactics for interdepartmental collaboration and operational excellence.
- Coordinates tasks and responsibilities amongst collaborating team members, ensuring clarity and accountability.
- Provides technical support, assistance, guidance, and direct support for relevant team members to promote and protect effective operations, implementation of related objectives, and to be a source of information to senior leadership, encouraging innovation and continuous improvement.
- Supports the establishment of key performance indicators (KPIs) and metrics to monitor program effectiveness and impact, conducting regular evaluations and assessments to identify areas for improvement and implement corrective actions.
- Prepares and presents comprehensive reports on program status, outcomes, and recommendations to senior management.
- Seeks and prepares proposals for external funding possibilities from governmental, foundation or other private sources in collaboration with program leadership.
EDUCATION/EXPERIENCE REQUIRED:
- Bachelor's degree in public health, social work, health administration or closely related field. Master's degree preferred.
- Two (2) years of supervisory experience preferred.
- Three to five years progressive administrative experience preferred.
- Two (2) years of experience working directly in community-based setting on behalf of under resourced or vulnerable population(s).
- Two (2) years of experience working within healthcare operations, including working knowledge of finance models, ROI analysis, regulatory, etc.
- Successful completion of an Administrative Fellowship Program (i.e., Henry Ford Health Fellowship Program) may be considered in lieu of the above experience.
- Demonstrable understanding of community/public health principles and theories.
- Ability to develop and maintain effective relationships with medical, administrative, support staff, patients, and the public.
- Ability to set priorities and avoid crisis. Ability to remain calm and poised in urgent situations.
- Skilled in preparation and presentation of reports to various committees and groups.
- Skilled in effective planning, both strategic and short-term, and organizing attributes.
- Skilled in exercising initiative, judgement, problem solving, and decision making.
- Skilled in analysis development of comprehensive reports and interpretation of complex data.
- Requires excellent organization, written and verbal communication, interpersonal and relationship building skills.
- Knowledgeable in organization policies, procedures, systems, and objectives; fiscal and team-building techniques and applications; and healthcare administration, including managed care administration, regulatory management, government regulations, and computer systems and applications.
- The ability to manage processes, promote process improvement, work effectively as part of a team, motivate others, and work comfortably with diverse populations are also key.
CERTIFICATIONS/LICENSURES REQUIRED:
Must meet or exceed core customer service responsibilities, standards and behaviors as outlined in the Henry Ford Health Customer Service Policy
Must practice the customer skills as provided through on-going training and in-services.
