Job Summary

We are seeking a compassionate and dedicated HHA or CNA to provide exceptional in-home care to clients across diverse needs. As a vital member of our caregiving team, you will deliver personalized support that promotes independence, safety, and comfort. Responsibilities

  • Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using equipment such as Hoyer lifts.
  • Monitor and observe patients' conditions regularly, documenting vital signs, symptoms, and changes in health status accurately within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems.
  • Administer medication reminders following prescribed schedules and protocols while maintaining compliance with HIPAA regulations to ensure patient confidentiality.
  • Provide specialized care for clients with dementia or Alzheimer's disease, including memory care strategies and behavioral management techniques.
  • Support clients with medical procedures such as catheter care, wound dressing changes, and hospice-related needs under supervision or as trained.
  • Prepare nutritious meals tailored to dietary restrictions and assist with feeding when necessary, ensuring proper nutrition and hydration.
  • Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment to promote overall well-being.
  • Assist with medical documentation and record keeping to ensure accurate tracking of patient progress and compliance with healthcare standards.
  • Collaborate with healthcare professionals to coordinate care plans for clients with developmental disabilities or long-term care needs.
  • Experience with Stroke Patients is a huge Plus.

Requirements

  • Valid certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA).
  • Proven experience in home health care or assisted living environments; nursing home experience is highly valued.
  • Demonstrated ability to work effectively with seniors, individuals with disabilities, or those requiring memory or hospice care.
  • Strong knowledge of medical terminology, medication administration protocols, and patient safety procedures.
  • Current CPR certification; additional certifications such as hospice care training or catheter care are a plus.
  • Ability to lift patients safely using assistive devices like Hoyer lifts; physical stamina for long shifts is essential.
  • Excellent observational skills for patient monitoring and reporting changes promptly.
  • Compassionate attitude combined with excellent communication skills to build trusting relationships with clients and their families.
  • Flexibility to work live-in shifts or flexible hours based on client needs; reliable transportation is preferred.
