Spiritual Care Counselor

Summary of Duties and Responsibilities: The Spiritual Care Counselor ensures provision of appropriate spiritual services to HOF patients, families and other loved ones. Coordinates and provides liaison between HOF and the religious community.

Reporting Relationship: Counseling Services Manager
Position Type: Full-time; benefitted; Non-Exempt
Job Details and Essential Functions:
- Conduct or participate in the memorial service(s) for employees, facilities, and family members of Company's Bereavement Program as required or requested
- Provide direct pastoral interfaith care to Hospice patients and families including spiritual assessment, crisis intervention, counseling and linkage with spiritual resources
- Assess spiritual needs of patients/families/caregivers in a timely manner, and provide consultation. Contact patient/family's preferred spiritual advisor to ensure ongoing involvement and to invite participation in interdisciplinary care planning when requested by team member or patient/family member
- Develop and document plan for spiritual care, in conjunction with the interdisciplinary team when requested by team member or patient/family member
- Evaluate and revise plan for spiritual care of patients/families/caregivers with the interdisciplinary team on an ongoing basis
- Maintain records of spiritual care services utilization and related activities for quality assurance, program development and policy and procedure review and revision
- Provide consultation, education, and support to the interdisciplinary team on spiritual care. Participate in staff meetings, department meetings, team meetings, briefings, in-services, committees and other related activities as needed at various workplace settings as requested (SNF etc.)
Knowledge and Skills
Required
- Comfort in an interfaith setting. Ability to accept different lifestyles, cultures, beliefs and values.
- Ability to network with community clergy and congregations
- Understanding of Hospice philosophy, principles of death/dying. Compliance with accepted professional standards and practices.
- Must have the ability to function autonomously and possess effective time management skills
- Experience inter-acting with families and patients during the transition, dying and bereavement periods.
- Intermediate experience with MS Office (Outlook, Word, computer software)
Education & Experience
Required
- Graduate of accredited seminary or school of theology (B.D., M.Div. or equivalent theological degree), or degree in counseling with some theological training, or equivalent experience
- Experience inter-acting with families and patients during the transition, dying and bereavement periods.
- Three years of experience working with death and dying
- One year of Hospice/home health experience
- Knowledge of and commitment to hospice philosophy of care
Desired
- Five years of experience working with death and dying
- Three years of hospice/home health experience
License / Certifications
Required
- Possess and maintain valid California driver's license, a reliable automobile and evidence of appropriate liability insurance
