General Summary

This is a full time position under the direct supervision of the SAS Program Manager. The CPSS functions as part of community based transdisciplinary team serving adults with severe and persistent mental illness not reached by traditional services. The general responsibility of the position is to provide case management interventions helping clients achieve their personal recovery goals.

Principal Duties and Responsibilities:

  1. Participate in ACT training

  2. Provide in vivo case management for ACT clients including coordinating and monitoring services. Advocate on client's behalf for resources and access to services.

  3. Act as a liaison between client and the community, building relationships with families, community agencies, and employers.

  4. Provide ongoing assessment and encouragement in regards to Activities of Daily Living. Monitor symptoms and self-management of symptoms that might interfere with a person achieving their highest level of functioning and use crisis situations to establish a relationship with disengaged clients.

  5. Enhance interpersonal and community coping skills through individual interventions including adapting to home, school, and work environments and help clients advocate for themselves and facilitate skills for self-management of crisis.

  6. Assume the role of an informal specialist in the areas of benefits and housing.

  7. Complete all necessary documentation and statistical reports within required timelines, achieve an annual average of at least 90% on initial score of clinical record review.

  8. Administers outcome ratings on all primary cases based on outcome schedule.

  9. Administers Client Satisfaction Surveys to all primary cases, annually.

  10. Maintains and completes all required documentation in the integrated clinical records as indicated by the Clinical Records Manual, and will meet all required timelines for completion of documentation. Episodes will be finalized within five days of service delivery

  11. Completes Incident Reports, Crisis Alerts, and Hospitalization Forms as appropriate.

  12. Demonstrates familiarity with agency's computer system

  13. Participates in and is on time for all mandatory staff meetings, case conferences, and treatment meetings and meets one hour weekly for individual supervision.

  14. Achieve 100% of the productivity designated for the position.

Minimum Qualifications:

An Associate's or Bachelor's Degree in the human services field. Experience in providing services is preferred. Access to an operational vehicle and a valid driver's license is required. Full coverage auto insurance with 100,000/300,000 liability policy coverage must be maintained
