Job Overview:

Ramp Health is seeking a full-time Certified Diabetes Care and Education Specialist (CDCES) to join our growing team This is a virtual position with flexibility, providing both nutrition counseling and diabetes education to participants across our national programs. Licensure in Pennsylvania and Delaware is strongly preferred.

Key Responsibilities:

  • Conduct virtual one-on-one diabetes care and education sessions, integrating medical nutrition therapy and self-management principles
  • Provide evidence-based nutrition counseling for individuals with diabetes, prediabetes, and related conditions
  • Offer digital follow-up and between-session support to enhance participant engagement and outcomes
  • Facilitate educational webinars, group sessions, and cooking demonstrations to promote healthy habits and nutritional awareness
  • Collaborate with Ramp Health's interdisciplinary team — including wellness coaches, nurses, and RDs — to ensure comprehensive care
  • Maintain accurate, timely documentation in compliance with HIPAA and company standards

Required Qualifications:

  • Certified Diabetes Care and Education Specialist (CDCES) credential
  • Registered Dietitian credential through the Commission on Dietetic Registration (CDR)
  • Active state licensure (licensure in Pennsylvania and Delaware preferred)
  • Minimum 2 years of experience providing diabetes care and nutrition counseling
  • Demonstrated skills in time management, client communication, and motivational interviewing
  • Ability to work independently in a virtual care environment
  • Strong commitment to cultural humility and patient-centered care

Preferred Qualifications:

  • 3+ years as a Certified Diabetes Care and Education Specialist
  • Credentialed with major insurers such as Independence Blue Cross, Highmark, Aetna, United Healthcare, and Cigna
  • Additional certification such as Board Certified Specialist in Obesity and Weight Management (CSOWM)
