  • Schedule: Full-time, salaried (Monday–Friday with field support as needed)
What you'll do
  • Direct and coordinate patient care across RN, therapy, and aide teams

  • Lead QAPI, audits, and IDG meetings to keep quality survey-ready

  • Coach field nurses on clinical judgment and OASIS accuracy

  • Oversee onboarding and ongoing development for all new clinicians

  • Partner with Executive Director on outcomes, rehospitalization, and staff ratios

  • Back up field visits only when needed—your focus is leadership, not endless travel

Must-haves
  • Active Illinois RN license

  • 2+ years in home health or hospice care

  • 1+ year of supervisory or charge nurse experience

  • Strong grasp of OASIS and Medicare documentation

  • Familiarity with CHAP or other accreditation standards
