Contract Details:
- Contract Type: Permanent
- Shift: Days (Mon–Fri, 8 AM–5 PM)
- Hours/Week: 40
- Pay Rate: $22.39 – $28.09 per hour
Role Overview:
As a Home Care Coordinator, you'll support the interdisciplinary team by coordinating unskilled home-based services such as meal delivery, caregiver scheduling, and DME (durable medical equipment) logistics. You'll conduct home safety evaluations, maintain documentation, and advocate for participants' needs with compassion and professionalism.
Responsibilities:
- Coordinate delivery of meals, caregiver services, and DME to participant homes
- Conduct home safety evaluations during enrollment, address changes, and semi-annual reviews
- Document service delivery and progress in EMR systems
- Monitor quality and timeliness of services from external vendors
- Communicate regularly with participants, families, and care providers
- Support nurses with logistics and coordination
- Conduct monthly visits for homebound and RCFE participants
- Deliver supplies and assist with participant surveys
- Participate in IDT meetings and staff training
- Maintain compliance with HIPAA, OSHA, and internal policies
Qualifications:
- License/Certifications:
- CNA certification
- California Department of Public Health Home Health Aide certificate (HHA)
- CPR and First Aid certification from the American Heart Association
- Valid California Driver's License
- Experience: Minimum 1 year working with frail or elderly populations
- Education: High school diploma or equivalent with required certifications
Preferred Skills:
- Strong coordination and communication abilities
- Familiarity with EMR systems and Microsoft Office tools
- Understanding of HHA scope of practice
- Ability to work independently and within a team
- Compassionate, culturally sensitive approach to care
