Position Overview

We are seeking a dedicated and compassionate Social Services Director to join our team. The ideal candidate will play a vital role in ensuring the psychosocial well-being of our residents by addressing their emotional, social, and psychological needs. This position requires a strong commitment to advocacy, effective communication, and fostering positive relationships with residents, families, and staff.

Responsibilities
  • Assess and address the psychosocial needs of residents upon admission and throughout their stay.
  • Develop and implement individualized care plans in collaboration with interdisciplinary teams.
  • Serve as a liaison between residents, families, and staff to facilitate communication and resolve concerns.
  • Provide emotional support, counseling, and crisis intervention to residents and families.
  • Coordinate discharge planning and ensure a smooth transition for residents returning to the community or moving to another care setting.
  • Advocate for residents' rights and ensure compliance with federal, state, and local regulations.
  • Maintain accurate documentation of all assessments, care plans, and interventions.
  • Organize and facilitate support groups and educational programs for residents and families.
  • Stay updated on community resources and connect residents/families to appropriate services.
Qualifications
  • Current SSD Certificate or willingness to obtain.
  • Previous experience in a long-term care or healthcare setting is highly desirable.
  • Strong knowledge of federal and state regulations related to long-term care facilities.
  • Excellent communication, organizational, and interpersonal skills.
  • Ability to work collaboratively as part of an interdisciplinary team.
  • Compassionate and patient-focused approach to care.
