Madrigal is a biopharmaceutical company focused on delivering novel therapeutics for metabolic dysfunction-associated steatohepatitis (MASH), a serious liver disease that can progress to cirrhosis, liver failure, need for liver transplantation and premature mortality. Madrigal's medication, Rezdiffra (resmetirom), is a once-daily, oral, liver-directed THR-β agonist designed to target key underlying causes of MASH. Rezdiffra is the first and only medication approved by both the FDA and European Commission for the treatment of MASH with moderate to advanced fibrosis (F2 to F3). An ongoing Phase 3 outcomes trial is evaluating Rezdiffra for the treatment of compensated MASH cirrhosis (F4c).

Role Summary
The Senior Director, GHEOR (Field) will advance the strategic objectives of GHEOR, Global Medical Affairs (GMA), and the broader organization by driving patient access through optimal pricing and reimbursement for Madrigal Pharmaceuticals' medicines, with a primary focus on Rezdiffra.
This role directly impacts the achievement of global business objectives by providing strategic leadership and delivering robust evidence generation to demonstrate the value of Madrigal's therapies in liver health. The Senior Director will lead the development and execution of real-world data (RWD) and real-world evidence (RWE) strategies that support Rezdiffra and the broader Madrigal portfolio, operating within a federated model that enables cross-enterprise collaboration and service delivery.
The Senior Director will, in coordination and alignment with the Field Medical and Health Systems Teams, and the Market Access Teams (Payer and KAD teams) lead HEOR engagements with national and regional payers and large health systems critical to supporting and delivering the value proposition for Madrigal's products. This individual will be a leader in economic and public health value propositions for US payer customers and will be largely responsible for generating evidence (RWE, comparative effectiveness, PRO's, productivity, etc.) to inform population health decision makers with respect to the value of Madrigals product portfolio. The Senior Director will work closely and compliantly with HQ HEOR, Medical, Commercial, Market Access, Legal and Compliance, by providing HEOR leadership on key matters pertaining to medical, commercial, and access initiatives with population health decision making. Additionally, the Senior Director will play a keep role in building and maintaining Thought leader (TL) and population health decision makers, to support the medical and HEOR strategy for Madrigals medicines.
This position is field-based and customer-facing, with an expectation of regular in-person engagement. The role follows a hybrid model, based in the U.S. (Midwest/West Coast), with an anticipated on-site or customer-facing presence three days per week. All incumbents are expected to comply with applicable Madrigal and divisional policies and procedures.
Key Responsibilities And Primary Activities
Analytical Expertise and Technical Skills
Demonstrate strong analytical background in biostatistics, public health, or epidemiology with hands-on experience in real-world data (RWD) analysis
- Leads collaborative customer outcomes research projects, provides input on HQ HEOR projects, and assists in the development and execution of HEOR/market access relevant activities (e.g., payer advisory boards, consultant, and HEOR projects).
Provides impactful feedback/information to internal stakeholders on emerging payer formulary/coverage and reimbursing trends.
Real-World Evidence (RWE) and HEOR Research
Support real-world research projects in RWE and HEOR to generate actionable insights that inform business and clinical strategy
- Develops and maintains clinical acumen related to disease-states of interest for Madrigal's pipeline and marketed products.
- Identifies and engages key stakeholders within the payer verticals (Insurers, PBMs, GPOs, Specialty Pharmacy, and Provider Networks) with key HEOR data on Madrigals medicines.
- Establishes and maintains scientifically credible peer-to-peer scientific & collaborative research relationships with national & regional payers and managed care (e.g. Health plan P&T members, and formulary decision bodies).
Establishes and maintains scientifically credible peer-to-peer scientific relationships with academic researchers in HEOR & policy.
Cross-Functional Collaboration and Communication
Collaborate effectively with cross-functional stakeholders to define research questions, shape analyses, and communicate findings in a clear and impactful manner
- Present study results at scientific conferences and publish research in peer-reviewed journals to enhance scientific visibility and credibility
- Provides impactful feedback/information to internal stakeholders on emerging payer formulary/coverage and reimbursing trends.
- Represents Madrigal on HEOR issues at national/global forums, conferences, and professional associations, including monitoring and communicating activities.
Works with internal partners, e.g. market access, commercial and medical teams, and interacts with external consulting KOLs for collaborative HEOR studies.
Continuous Learning and Industry Awareness
Stay current with emerging trends, best practices, and advancements in RWE and HEOR methodologies
- Contribute to continuous improvement of analytical tools, research processes, and data standards across the organization
Champions FDAMA 114 content development and approval for use with payers, and train internal teams (field medical & Account Executives) for complaint use with relevant audiences.
Budget Oversight and Resource Management
Ensures project milestones and timelines are being executed against SOW's
Manage project deliverables and purchased orders being paid in line with budget
Team Leadership & Development
Foster continuous development through mentoring and training
Compliance & Quality
Ensure all activities comply with company SOPs, industry standards, and applicable regulations
Uphold the highest ethical standards in all external engagements
Travel Requirement
Willingness to travel both locally and internationally up to 50% for conferences, various corporate locations, and stakeholder meetings, including overnight trips

Required Qualifications, Skills & Experience
- MS/PhD in biostatistics, public health, epidemiology, or a similar field
- Strong experience working at or directly with payer and health systems on HEOR initiatives including collaborative research or policy initiatives
- 15+ years' of relevant experience in HEOR with a proven track record of delivering results
- Knowledge of R or SAS programming
- Familiarity with common commercially available RWD data assets encompassing claims & EHR's
- Experience in delivering ad-hoc and full protocol-driven observational studies
- Ability to program and/or validate code across multiple languages (e.g., SAS and R)
- A strong track record of publications
- Strong communications skill and interpersonal skills with ability to engage multiple internal and external stakeholders
- Exceptional written and verbal English communication skills
- Demonstrated ability to assess and prioritize projects based on business value
- Execution-focused mindset with emphasis on efficiency and results
- Strong project management skills in contracting, budgeting, and vendor oversight; adept at multitasking under shifting priorities

Preferred Attributes
- In-depth knowledge of liver disease in the RWD space; experience in cardiometabolic health or Metabolic Dysfunction-Associated Steatohepatitis

Base salary is determined by several factors including the candidate's qualifications, skills, education, experience, business needs, and market demands. As of the date of this posting, a good faith estimate of the current pay scale for this position, applicable to all candidates, is $272,000 - $332,000 per year. We comply with all applicable minimum wage laws.
