Job Overview

We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our healthcare team. The ideal candidate will have a strong commitment to providing high-quality patient care and will possess a diverse skill set that includes experience in various medical settings. As an LVN, you will play a crucial role in supporting patients' health and well-being through direct care, assessment, and collaboration with healthcare professionals.

Duties

  • Provide direct patient care, including administering medications and treatments as prescribed.
  • Conduct patient assessments and monitor vital signs to ensure optimal health outcomes.
  • Assist in the development and implementation of individualized care plans.
  • Perform IV infusion, catheterization, and other nursing procedures as needed.
  • Maintain accurate medical documentation and records in compliance with HIPAA regulations.
  • Collaborate with interdisciplinary teams to coordinate patient care across various settings such as hospitals, nursing homes, assisted living facilities, and home care.
  • Educate patients and their families on health management, medication administration, and disease prevention.
  • Support patients with developmental disabilities, dementia care, Alzheimer's care, and other specialized needs.
  • Participate in triage processes in emergency or acute care settings.

Qualifications

  • Current Licensed Vocational Nurse (LVN) license in the state of practice.
  • Experience in critical care, long-term care, assisted living, or home health preferred.
  • Proficiency with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks is highly desirable.
  • Knowledge of ICD-9/ICD-10 coding and medical terminology.
  • Familiarity with acute pain management techniques and medication administration protocols.
  • Experience working with diverse populations including geriatrics, pediatrics, and individuals with disabilities is a plus.
  • Strong assessment skills with the ability to recognize changes in patient conditions promptly.
  • Excellent communication skills for effective interaction with patients, families, and healthcare teams. Pay: $ $39.69 per hour
