Job Summary:

The Medical Coordinator supports the daily operations of services provided to individuals living in group homes. This role is primarily responsible for scheduling appointments (medical, dental, therapy, etc.), ensuring all required documentation is properly completed, filed, and maintained, and providing general administrative support to program staff. The ideal candidate is highly organized, detail-oriented, and comfortable working in a team environment focused on supporting individuals with developmental or behavioral health needs.

Job Type: Full-time

Schedule: Monday - Friday: 8:30 AM - 5:00 PM

Payrate: $75,000.00

Location: Must Travel to Lehigh County, Wayne County, and Carbon County

Key Responsibilities:

  • Schedule and confirm medical, dental, therapy, and other essential appointments for residents.
  • Coordinate transportation arrangements as needed.
  • Maintain accurate and up-to-date appointment logs and calendars.
  • Ensure all documentation related to appointments, care, and services is accurately completed, signed, and submitted in a timely manner.
  • Monitor and follow up on missing or incomplete documentation.
  • File and maintain confidential records in compliance with organizational policies and state/federal regulations.:
  • Assist program staff with the completion of forms, reports, and internal communications.
  • Maintain up-to-date contact lists and directories for service providers.
  • Provide assistance during audits and inspections by ensuring documentation is organized and available.
  • Support compliance efforts by helping staff adhere to documentation standards and timelines.
  • Collaborate with case managers, direct support professionals, and health professionals to ensure continuity of care.
  • Serve as a liaison between group home staff, residents, and outside providers to facilitate seamless communication and service delivery.
  • Advocate for the timely scheduling and completion of necessary services for residents.

Qualifications:

  • Associate's or Bachelor's degree in human services, healthcare administration, or related field preferred.
  • At least one year of experience in an administrative or support role, preferably in a healthcare, group home, or social services setting.
  • Strong organizational and time-management skills.
  • Ability to manage multiple priorities and meet deadlines.
  • Excellent communication and interpersonal skills.
  • Proficient in Microsoft Office Suite (Word, Excel, Outlook).
  • Knowledge of HIPAA and confidentiality standards.
  • Ability to work independently and as part of a team.
