Overview

We are seeking a committed, experienced and skilled Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will have a strong background in patient care, with experience across various medical setting which may include family practice, long-term care, assisted living, hospitals, and outpatient clinics. The LPN will play a vital role in providing high-quality nursing services, supporting patient recovery, ensuring compliance with healthcare standards and providing oversight of the MA team. This position offers an opportunity to work in various clinical environments, utilizing extensive nursing skills and specialized knowledge to improve patient outcomes.

Duties

  • Administer medications and injections accurately following physician orders and established protocols
  • Perform patient assessments, including vital signs, physical examinations, and documentation of health history via the EHR.
  • Assist with diagnostic evaluations such as EKG's, specimen collection, processing, and laboratory testing
  • Provide oversight, direction, guidance and teaching to the MA team at the discretion of the Practice Manager (or designee).
  • Provide assistance to the provider for in office procedures
  • Support and educate patients with complex medical needs.
  • Operate EMR/EHR systems like eClinicalWorks, Epic, Cerner, and Athenahealth, for accurate medical documentation
  • Conduct patient education on medication administration, health coaching, discharge planning, and post-acute care instructions
  • Collaborate with multidisciplinary teams across the spectrum of healthcare settings to support the care of patient in the practice.
  • Ensure infection control practices are maintained according to NCQA standards and OSHA guidelines.
  • Function as a vaccine coordinator for the VFC programs.

Requirements

  • Valid Licensed Practical Nurse (LPN) license in DC and Maryland.
  • Proven experience in diverse clinical settings such as primary care, acute care, hospital and outpatient environments
  • Familiarity with ICD-10 coding systems for documentation purposes
  • Experience with EMR/EHR systems including eClinicalWorks Epic, or Cerner, is highly preferred
  • Experience with VFC programs and protocols a definite plus and highly preferred
  • Knowledge of physiology, anatomy, pharmacology terminology, and diagnostic evaluation techniques
  • Ability to perform venipuncture, phlebotomy, including sterile technique
  • Strong patient assessment skills with the ability to work effectively with people of all ages including pediatrics (children/toddlers/neonates), seniors (geriatrics), and individuals with disabilities or developmental challenges
  • Excellent communication skills for working with patients and multidisciplinary teams; experience working with people with disabilities or complex needs is a plus
  • Certification in Basic Life Support (BLS) required; additional certifications such as ACLS or PALS are advantageous
  • Ability to work independently while adhering to healthcare regulations such as HIPAA and infection control policies
