Position Summary
Under the supervision of the Tailor Made Program Director, the Case Manager Coordinator manages case management services, supervises staff, and provides direct support to youth clients. The role ensures program quality, compliance, and alignment with organizational and funder requirements. The Coordinator fosters a safe and inclusive environment, supports program events, contributes to staff training and development, and assists with Urban Services' annual Community Needs Assessment as part of Community Action Agency requirements.
Key Responsibilities
Program Management & Operations
Oversee daily operations of the Tailor Made case management program.
Ensure all forms, tools, and documentation for clients and life skills groups are current and accessible.
Prepare and submit monthly reports, including CMS narratives and progress updates.
Lead weekly staff meetings and communicate key information to absent team members.
Ensure timely and accurate data entry in Salesforce and NPI systems.
Case Management & Client Services
Maintain a caseload of 5–10 youth clients (ages 11–18).
Conduct intakes, assessments, and case plans following program protocols.
Support youth in setting and achieving personal, academic, and social goals.
Connect clients and families to appropriate community resources and services.
Maintain up-to-date, confidential client files and case notes.
Track and report participant demographics, attendance, and progress data.
Staff Supervision & Development
Co-supervise and support all case management staff.
Provide training, coaching, and feedback to ensure consistent service quality.
Promote staff growth through ongoing professional development opportunities.
Program Quality & Compliance
Monitor and assess program quality; implement improvement strategies as needed.
Ensure compliance with YGSF, district, funder, and organizational policies.
Uphold safety standards and best practices in all youth interactions.
Oversee contract management and adherence to fiscal and reporting requirements.
Coordination & Collaboration
Work closely with the Program Director to align case management practices with organizational goals.
Attend School Crisis Support Initiative meetings to support coordination with DCYF and other partners.
Assist in planning and implementing Tailor Made program events.
Qualifications
Preferred
Associate or Bachelor's degree in Child & Adolescent Development, Social Work, or a related field.
Experience working in school or youth program settings.
Bilingual proficiency (Spanish, Cantonese, Mandarin, Korean, Vietnamese, or other languages).
Work Environment & Physical Demands
Work occurs primarily in schools and community settings, with a combination of office and fieldwork.
Frequent interaction with youth, families, and partners.
Requires occasional standing, walking, and lifting up to 25 lbs.
Monday – Friday during school hours with some flexibility to work occasional evenings based on program needs.

Salary Description
The salary range for this position is between $68,640 and $71,000, depending on experience and qualifications. This salary range is based on industry standards, salary benchmarking, and is aligned with the approval of grant funds designated for this role.
