The Specialist-Emergency Department (ED) Outreach works with a vulnerable population that has a high utilization of the Emergency Department.  The ED Outreach Specialist works in conjunction with patients, team members, hospital staff and community partners to develop and implement strategies that improve the health of individuals who are identified with high utilization of emergency department services. Organizes, collects, reviews and reports health and social information through member home visits and phone outreach, while demonstrating multicultural sensitivity and effective communication skills.

Total of $5,000 Sign on Bonus for Eligible External Candidates

Required Qualifications:

  • Bachelor's Degree Social Work, Social Sciences, or human services field preferred.
  • Within 30 days of hire: American Heart Association (AHA) BLS for Health Care Provider completion required or obtain within one month of hire
  • Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information.
  • A Department of Human Services (DHS) criminal background check may be required for this position based on location. Fingerprinting may also be required
  • 2 years Experience working with diverse vulnerable populations, community and/or health care organization experience.

Preferred Qualifications:

  • Direct care experience in a residential and/or outreach position serving vulnerable populations.
