Job Summary

We are seeking a compassionate and dedicated Home Health Aide to provide essential support to individuals in their homes or community settings. The ideal candidate will assist clients with daily living activities, ensuring their comfort and well-being while promoting independence. This role requires a strong commitment to patient care, effective communication skills, and the ability to work collaboratively with healthcare teams.

CNA, GNA , HHA STATE LICENSES ARE WELCOMED

Shift time Monday to friday 6:30am to 11:30am

Shift : Weekends 9am to 3pm Sat & Sun

Duties

  • Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting.
  • Provide companionship and emotional support to clients, fostering a positive environment.
  • Monitor and document vital signs, including blood pressure, pulse, and temperature.
  • Administer medications as prescribed and ensure proper medication management.
  • Assist with mobility using equipment such as Hoyer lifts or wheelchairs.
  • Perform light housekeeping tasks including cleaning, laundry, and meal preparation.
  • Support clients with memory care and dementia care strategies.
  • Maintain accurate medical documentation in compliance with HIPAA regulations.
  • Collaborate with nursing staff and other healthcare professionals for comprehensive patient care.
  • Provide specialized care for individuals with developmental disabilities or chronic illnesses.

Skills

  • Strong patient observation skills to identify changes in client conditions.
  • Experience in assisted living or nursing home environments is preferred.
  • Knowledge of EMR/EHR systems for effective medical documentation.
  • Familiarity with medical terminology and CPR certification is advantageous.
  • Experience in catheter care, hospice care, and long-term care settings is beneficial.
  • Ability to work effectively with individuals requiring Alzheimer's care or memory support.
  • Proficient in home & community care practices, ensuring quality caregiving standards.
  • Excellent communication skills for interacting with patients, families, and healthcare teams.
  • Capable of performing light cleaning and cooking tasks as part of daily responsibilities.
