About the Role/Dept:

The Case Management Team Lead at Sinai Hospital provides feedback on daily care management processes, productivity, and staffing issues to the Manager of Care Management while balancing an assigned CM workload and provide mentoring to CM staff to ensure compliance with discharge planning standards. The CM Team Lead act as a first line resource for the CM staff and the first point of contact for issues related to daily CM activities.

Key Responsibilities:

• Oversees the daily activities ensuring department standards are met, and provides real-time mentoring, advisement, and education to CM staff following CM processes.
• Works with the CM Manager on the day-to-day activities of the department, including but not limited to assisting in creating staff schedules and ensuring adequate coverage.
• Quality and productivity monitoring of staff work and documentation in EMR and EMR tools. Guide staff toward desired outcomes, setting high-performance standards and delivering quality services.

Requirements:

• Education: Master of Social Work
• Licensure: LMSW, LCSW or LCSW-C
• Certification: Case Management certification preferred
• Experience: 3-5 years of acute care hospital Case Management experience required
