Position Overview: Clinical Lead / Care Partner Manager

We are seeking an experienced, mission-driven Clinical Lead to manage and mentor our Care Partner (nursing) team and help continuously refine and evolve the PRISM service.

This role blends clinical expertise, leadership, and hands-on patient interaction. You'll support complex post-stroke patients virtually, guide nurses through clinical decision-making, and collaborate closely with product and operations teams to enhance workflows, protocols, and the overall care model.

The Care Partner Manager will be expected to monitor a small cohort of patients for the first 6months of the position, such that they are able to gain hands-on experience of what care partners do. After that the role will shift to focus more on clinical support, audit and management of the nursing team and assisting with design and evolution of the PRISM product,

What You'll DoClinical Leadership & Team Management

  • Lead, mentor, and support a team of Care Partners (RNs/LPNs)
  • Oversee clinical quality, adherence to protocols, and escalation pathways
  • Participate in hiring, onboarding, and ongoing training of nursing staff
  • Identify opportunities to elevate care for individual patients
  • Provide real-time clinical guidance on complex post-stroke cases
  • Support staffing, scheduling, and workflow optimization as the team scales

Patient Care & Clinical Excellence

  • Conduct virtual check-ins to review BP readings, symptoms, and recovery progress
  • Perform remote motor assessments and identify red flags early
  • Analyze patient trends and guide interventions using established protocols
  • Coordinate with hospitals, rehab teams, and community partners to ensure seamless care
  • Ensure timely, accurate clinical documentation

Shaping the PRISM Program

  • Partner with product, operations, and leadership to refine clinical workflows
  • Contribute feedback that helps improve our app, dashboard, and monitoring tools
  • Help design scalable processes as we grow across NY and beyond

What We're Looking For

  • Registered Nurse (RN) or Advanced Practice Registered Nurse (APRN) with an active NY license (required)
  • Compact license (preferred)
  • 10+ years of total nursing experience
  • 5+ years of experience caring for stroke patients (acute, post-acute, neurology, home health, or stroke program)
  • Experience leading or mentoring clinicians strongly preferred
  • Comfortable with technology and virtual care environments
  • Excellent clinical judgment, communication, and problem-solving skills
  • Bilingual (especially Spanish) is a plus

Job Type: Full-time

Pay: $80, $120,000.00 per year
