Job Description

Proactive Hospice Care is seeking a caring and experienced Licensed Medical Social Worker (LMSW) to join our team. As a critical part of our interdisciplinary group, the LMSW will offer emotional, social, and practical support to patients and their families. This role focuses on enhancing the quality of life for patients by addressing their psychosocial and emotional needs, ensuring they receive the resources and care they deserve.

Key Responsibilities

  • Conduct psychosocial assessments to identify patient and family needs.
  • Develop individualized care plans in collaboration with the hospice team.
  • Provide emotional and grief counseling for patients and their families.
  • Facilitate advance care planning and end-of-life decision-making.
  • Connect families with community resources for financial, legal, and caregiving assistance.
  • Advocate for patients and families, ensuring their voices are heard in care decisions.
  • Document all assessments, interventions, and follow-ups accurately and on time.
  • Participate in interdisciplinary team (IDG) meetings and contribute to care planning.

Qualifications

  • Master's Degree in Social Work (MSW) from an accredited institution.
  • Active LMSW license in Michigan.
  • Minimum of 2 years of experience in hospice, healthcare, or a related field.
  • Knowledge of hospice philosophy, grief processes, and community resources.
  • Strong communication, interpersonal, and problem-solving skills.
  • Ability to work with diverse populations and demonstrate cultural sensitivity.
