Integrated Care Coordinator
Location: In-Person | State College, PA
About the Role
Pyramid Healthcare is seeking an Integrated Care Coordinator to support mental health and performance programs for student-athletes. The coordinator serves as a key point of communication and organization between sports teams and behavioral health providers to ensure comprehensive, athlete-centered care. This role is responsible for coordinating referrals, scheduling, follow-up, and case management to optimize each athlete's well-being and performance.
Essential Duties and Responsibilities
- Assist clients in accessing behavioral health services, including intake, referrals, and follow-up care.
- Coordinate appointments and communication between coaches, students, and care providers.
- Maintain updated knowledge of community resources, treatment options, and support programs.
- Provide around-the-clock availability through an on-call rotation to ensure 24/7 support.
- Collaborate with coaching staff, families, therapists, psychiatry, and school personnel to ensure continuity of care.
- Track progress and adherence to treatment plans, ensuring follow-through with scheduled services.
- Partner with sports teams and coaches to address specific needs.
- Exercise discretion and maintain confidentiality with all company and client information.
- Complete all required trainings.
- Perform other duties as assigned.
Required Qualifications
Education, Licensure, and Experience
- Master's Degree or higher from an accredited college in medicine, chemical dependency, psychology, social work, counseling, nursing (with a specialty in health administration or counseling education), public administration, business management, or another related field.
- Minimum of two years of experience in a human service agency, preferably in a mental health setting, including supervision, direct service, and program planning.
- Two to three years of relevant field experience preferred.
- Experience working with student-athletes and collaboration within a Sports Medicine or Athletic Department setting is highly desirable.
