Transitional Support Specialist

Job Description:
Under the supervision of the Life Skills Administrator or Adult Services Administrator, the Transitional Support Specialist provides hands-on support, personal care, and skill-building assistance to individuals with developmental and other disabilities. This includes helping individuals engage in daily living activities, social and recreational experiences, day services, and community involvement, all in alignment with their Individualized Support Plans (ISPs). The role emphasizes collaboration across teams, departments, and organizational boundaries to ensure integrated and person-centered service delivery.
As a TSS, you will help individuals lead self-directed lives, achieve personal goals, and actively participate in their communities. You'll also promote inclusive attitudes and behaviors that support meaningful community integration and personal growth.
Qualifications and Abilities:
- Strong visual and auditory perception to effectively read, write, complete required documentation, observe and assist individuals served, and operate assigned equipment or vehicles.
- Basic proficiency in using computers and mobile technology, including iPads, tablets, and smartphones.
- Sound judgment and reasoning skills, especially in situations that may impact the health, safety, or well-being of individuals served. Ability to organize tasks, manage workflow, and handle both abstract and concrete challenges.
- Capable of preparing clear, accurate, and meaningful reports.
- Excellent interpersonal skills with the ability to collaborate effectively across teams and departments.
- Willingness and ability to obtain and apply First Aid, CPR, and behavior/aggression management techniques appropriate to the needs of the individuals and program.
- Physical ability to intervene during behavioral crises to ensure the safety and well-being of individuals served.
- Strong verbal communication skills, including the ability to speak clearly and confidently in English, and to learn alternative communication methods to support individuals with varying needs.
