Job Summary:
Our Licensed Home Care Services Agency (LHCSA) is seeking a highly organized
Nursing Visit Scheduler and Clinical Support Specialist
to support our clinical operations. This role is responsible for coordinating and scheduling field nursing visits, maintaining accurate and up-to-date patient records in HHAeX EMR, and assisting the nursing department with audit preparation and compliance activities. The ideal candidate is detail-oriented, proactive, good communicator, and comfortable working in a fast-paced environment with multiple priorities and working knowledge of New York City geography—including boroughs, neighborhoods, and travel patterns. They must be confident using technology, running reports, managing spreadsheets.
Responsibilities:
- Schedule, confirm, and track all nursing visits, including SOC, supervisory, and clinical follow-up visits
- Clustering visits geographically using strong knowledge of NYC boroughs, neighborhoods, and travel patterns
- Maintain patient records in the HHAeX EMR, process hospital transfers and discharges following NYS DOH LHCSA requirements
- Communicate with field nurses and managers to ensure smooth workflow and timely service delivery
- Serve as administrative support to the clinical department, including organizing charts, uploading documents, and managing deadlines
- Assist with internal and external audits (DOH, OMIG, MLTC), including gathering documentation, compiling visit logs, and ensuring data accuracy
- Monitor email queues and task lists to ensure timely responses to inquiries and requests
- Provide excellent customer service when interacting with patients, families, MLTCs, and contracts
- Perform other duties as assigned to support clinical operations
Qualifications:
- Active New York State LPN license
- Bilingual Spanish/English required
- Minimum 1 years of home care experience preferred, 1 year of nursing experience required
- Strong communication, critical thinking, and problem-solving skills
- Valid driver's license and reliable transportation; travel throughout NYC required when visits need to be made
Desired Skills and Experiences:
- Strong working knowledge of NYC boroughs, neighborhoods, and surrounding areas to efficiently cluster and route nursing visits
- Strong computer proficiency (Excel, Word, PDF editing, EMR HHAeX platform)
- Ability to manage spreadsheets, create simple trackers, and handle data entry with high accuracy
- Fast and accurate typing skills
- Ability to scan, upload, and organize documents logically and efficiently
- Excellent communication skills and professional phone/email etiquette
- Strong attention to detail and ability to manage multiple priorities
- Experience in home care, LHCSA (preferred) or CHHA
Pay:
$24/hr-$26/hr
