What You'll Do

  • Provide 1:1 outpatient physical therapy in clients' homes under PT supervision.
  • Deliver functional, personalized, evidence-based treatments.
  • Educate clients and caregivers on safe movement and mobility strategies.
  • Collaborate with the team to create effective, client-centered care plans.

What We're Looking For

  • Licensed Physical Therapist Assistant (PTA) in Connecticut.
  • 3+ years of experience (SNF, traditional home health, or outpatient preferred).
  • Experience or interest in neuro rehab / Parkinson's Disease a plus.
  • Strong communication skills, clinical independence, and passion for helping older adults thrive.
