Duties

  • Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
  • Monitor patient health by observing vital signs such as blood pressure, pulse, respiration, and temperature; report any abnormalities promptly
  • Support medication administration following proper protocols and documentation standards in compliance with HIPAA regulations
  • Help with medical procedures such as catheter care, wound care, and hospice support as directed by healthcare professionals
  • Operate and maintain Electronic Health Record (EHR) and Electronic Medical Record (EMR) systems for accurate medical documentation and record keeping
  • Utilize assistive devices like Hoyer lifts and other mobility aids to ensure safe patient transfers and prevent injuries
  • Prepare nutritious meals tailored to dietary restrictions and assist with meal planning and preparation
  • Perform light housekeeping duties including cleaning, laundry, and maintaining a safe environment for clients
  • Provide companionship and emotional support to promote mental well-being, especially for clients with dementia or Alzheimer's disease
  • Document care activities meticulously in medical records while respecting patient privacy and confidentiality standards
  • Collaborate with healthcare teams to coordinate home & community care services that meet individual needs

Qualifications

  • Proven experience in home health care or long-term care environments; nursing home experience is highly valued
  • Strong knowledge of medical terminology, medication administration procedures, and patient observation techniques
  • Familiarity with EMR/EHR systems for accurate documentation of patient information
  • Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice care or dementia training are a plus
  • Experience working with people with developmental disabilities or memory care needs preferred
  • Ability to assist with ADLs using proper techniques and assistive devices like Hoyer lifts safely
  • Excellent communication skills for working effectively with clients, families, and healthcare professionals
  • Knowledge of HIPAA regulations to ensure confidentiality of patient information
  • Valid driver's license and reliable transportation may be required depending on assignment locations
