About the Role
As a Nurse Practitioner with Matrix, you’ll conduct various health assessments during in-home visits that last up to one hour. You will have the opportunity to:
- Perform adult and geriatric assessments, including medical history evaluation and treatment planning.
- Provide health education and facilitate referrals as necessary.
- Engage collaboratively with physicians, case managers, and family members.
- Implement care strategies directly in members' homes or Skilled Nursing Facilities.

Qualifications:
- A Master's Degree and relevant NP licensure with prescriptive authority.
- Certification in Adult, Adult Gerontology, Family, Emergency, or Acute Care specialty.
- Current BLS, ACLS, or CPR certification.
- Preferred 1 year of NP experience; new graduates are encouraged to apply
- Valid driver's license and adequate auto insurance coverage.
- Strong computer skills and the ability to navigate medical record software.
- Excellent communication skills, both verbal and written.
