About the Role

In this role, you will conduct comprehensive health assessments in members' homes or Skilled Nursing Facilities, with visits lasting up to one hour. Our providers:

• Conduct adult/geriatric assessments, reviewing medical history, diagnosing, offering treatment, providing health education, making physician and case management referrals, and documenting according to Matrix guidelines.
• Administer and order necessary point-of-care and lab-based testing.
• Collaborate with physicians, case managers, social workers, family members, and other medical personnel as appropriate.
• Work alongside Primary Care Physicians for patient education and follow-up.

• Consistency: Full-time providers typically work Monday to Friday, 8:00 to 5:30, covering assessments from Sunday to Saturday, 8 am to 7 pm.
• Competitive Pay: Earn an annual base salary ranging from $95,500 - $158,400 based on market analysis and experience.

Qualifications

• Master's Degree or equivalent experience and completion of NP licensure.
• Current NP licensure in the state of practice with prescription authority or ability to obtain such authority.
• AANP, AACN, or ANCC certification in Adult, Adult Gerontology, Family, Emergency, or Acute Care specialties.
• Current BLS, ACLS, or CPR certification.
• 1 year of NP experience is preferred; new graduates are encouraged to apply.
• Willingness to travel up to 15% of the time, including overnight trips.
• Valid state driver's license and proof of adequate automobile insurance.
• Proficient in computer skills and familiarity with employee health/medical record software.
• Excellent verbal and written communication abilities.
