Responsibilities:

  • Coordination of home health services to ensure they meet industry standards and our company policies.
  • Review and approve patient information submitted by licensed professionals during various care visits.
  • Facilitate communication among physicians, referral sources, patients, caregivers, and employees to ensure seamless care delivery.

Qualifications:

  • Graduation from an approved nursing school.
  • Active nursing license in your practicing state, compliant with the nurse licensure compact for your state.
  • At least two years of nursing experience, with one year specifically in home health or hospice care.
  • Familiarity with federal, state, and local home care regulations and guidelines.
  • Basic technology skills, including mobile device proficiency.

Preferred Experience:

  • Previous management experience.

Requirements:

  • Valid state driver's license and a dependable vehicle in good working condition.
  • Automobile liability insurance as required by law.
  • Able to drive safely in various weather conditions.
