Join our client's team as a Care Management Coordinator and play a vital role in achieving continuity of care and member satisfaction The Care Management Coordinator supports care management activities and the teams assigned to members, ensuring services are delivered effectively by healthcare providers and partners, ultimately achieving continuity of care and high member satisfaction. This role involves direct interaction with members through telephonic outreach or home visits and meticulous documentation of the member's plan for care and services. ***Candidates must be located in San Antonio to go onsite for meetings as needed***

salary: $20 - $21.46 per hour
shift: First
work hours: 8 AM - 5 PM
education: High School

Responsibilities

As a Care Management Coordinator, your daily activities will focus on facilitating and supporting the member care journey:

  • Coordinates care activities based on the established care plan/service plan. Works closely with healthcare and community providers, partners, and members/caregivers to accommodate necessary changes or progress in the plan.
  • Serves as a key support resource for various member and/or provider inquiries, requests, or concerns related to the care plan/service plan.
  • Communicates proactively with care managers, practitioners, and other relevant parties as needed to facilitate member services and ensure continuity of care and service delivery.
  • May assist in performing service assessments/screenings for members and documenting the member's specific care needs.
  • Supports documenting and maintaining comprehensive member records in accordance with state and regulatory requirements. Facilitates the distribution of necessary documentation to providers as needed.
  • Follows established standards of practice and policies that are compliant with contractual requirements, regulatory guidelines, and clinical standards.
  • Performs other duties as assigned and ensures compliance with all company policies and standards.
