San Bruno Skilled Nursing is a 50 bed Post-Acute facility, we believe in a patient-centered approach and are looking for Social Services Assistant to join our team.
Full Time
3 days Weekday, Saturday and Sunday
$25/Hr
Responsibilities:
- Collaborate with residents and their families to establish goals and develop a course of action that promotes self-care, independence, and overall well-being.
- Provide medically related social services to support the highest practicable physical, mental, and psychosocial well-being of each resident.
- Assess social, family, psychological, and emotional needs to develop comprehensive care plans addressing social services issues.
- Offer emotional support and guidance to residents and families facing challenges such as coping with physical disabilities, health-related fears, and grief associated with loss or death.
- Actively participate in discharge planning and coordinate with appropriate community agencies and referrals as needed.
- Interview residents, guardians, and sponsors to obtain required admission information and signatures
- Prepare identification records for residents according to established policies and procedures
- Maintain an accurate record of available beds
- Admit, transfer, and discharge residents in adherence to established policies and procedures
- Provide residents with admission information packets, including resident rights, notice of privacy practices, and admissions contract
- High school diploma or equivalent required.
- Experience in skilled nursing and familiarity with PointClickCare (PCC) preferred, but not required.
- Demonstrated ability to work effectively in a fast-paced environment and prioritize tasks efficiently.
- Excellent communication and interpersonal skills, fostering positive relationships with residents, families, and staff.
- Ability to evaluate social and family information, psychological, and emotional needs to assist in assessing social services needs as well as develop care plans for social services issues.
