About the Role
As a Nurse Practitioner or Physician Assistant with Matrix, you will play a critical role in providing comprehensive health assessments that last up to one hour. This unique opportunity allows you to gain deep insights into a member's health and well-being, something that is often missed during routine office visits.Your responsibilities will include:
- Conducting thorough adult and geriatric assessments, including medical history reviews, diagnoses, treatment plans, health education, physician referrals, and case management.
- Administering and ordering necessary point of care tests and lab work.
- Collaborating with physicians, case managers, social workers, and family members to ensure holistic care.
- Providing services at members' residences, which may include private homes or skilled nursing facilities.

Skills & Experience That Shine
- A Master’s Degree or equivalent experience and successful completion of NP or PA licensure.
- Current NP/PA licensure with prescriptive authority or readiness to obtain as necessary.
- Board certification from NCCPA, AANP, AACN, or ANCC in relevant specialties.
- Current BLS, ACLS, or CPR certification.
- At least 1 year of relevant NP or PA experience.
- Willingness to travel up to 80-90% of the time, including overnight trips.
- Valid driver's license and proof of adequate auto insurance.
- Strong computer skills and familiarity with medical record software.
- Excellent verbal and written communication abilities.
