About the Role

As a member of our team, you will conduct assessments that last up to one hour, reviewing and evaluating a member's health, medical history, and overall well-being. The Comprehensive Health Assessment (CHA) you perform is crucial for improving care quality and addressing multiple health gaps in a single visit. Your responsibilities will include:

• Performing adult and geriatric assessments that cover medical history, diagnosis, treatment, health education, and referrals, while accurately documenting according to Matrix guidelines.
• Ordering and administering point-of-care tests and lab work as necessary.
• Collaborating with physicians, case managers, social workers, family members, and other medical professionals.
• Providing ongoing patient education and follow-up care, in coordination with Primary Care Physicians (PCP).
• Delivering services in the member's home or in Skilled Nursing Facilities.

• Consistent Schedule - While assessments are conducted seven days a week, full-time providers typically work Monday to Friday from 8:00 AM to 5:30 PM.
• Competitive Compensation - Annual base salary ranges from $105,500 to $168,400, determined by market analysis and experience.

Skills & Experience That Shine

• A Master's Degree or equivalent experience with satisfactory completion of NP or PA licensure.
• Current NP/PA licensure in your state of practice, including prescriptive authority or ability to obtain it.
• NCCPA, AANP, AACN, or ANCC certification in specialties such as Adult, Adult Gerontology, Family, Emergency or Acute Care.
• Current BLS, ACLS, or CPR certification.
• At least one year of NP or PA experience.
• Willingness to travel 80-90% of the time, including overnight stays and weekends, across your state and potentially neighboring states.
• A valid state driver's license and proof of auto insurance.
• Proficiency in computer skills and familiarity with medical record software.
• Excellent communication skills, both verbal and written.
