Program Overview

Henry Street Settlement’s newest health and wellness initiative, CONNECT (Continuous Engagement between Community and Clinic Treatment), serves adults in the Lower East Side who face challenges related to mental health, housing instability, and employment barriers. CONNECT bridges traditional clinical care with community-based engagement, meeting clients where they are—literally and figuratively—to support recovery, stability, and empowerment. The program’s mission is to improve quality of life by promoting rapid access to care, sustained engagement, and holistic wellness. CONNECT’s flexible model integrates mental health treatment, peer support, housing and employment resources, and community outreach, emphasizing accessibility, cultural responsiveness, and long-term recovery.

Job Summary

The CONNECT Clinician provides individual, group, and family therapy within a hybrid clinical and community-based model. As part of an interdisciplinary team, the clinician helps clients achieve recovery and self-defined goals through person-centered, trauma-informed care. This role blends traditional clinical responsibilities—such as intake, assessment, and evidence-based treatment—with active fieldwork that includes home visits, off-site sessions, and collaboration with community partners. The clinician also supports clients with co-occurring mental health and substance use needs through integrated treatment approaches and wellness planning.

Qualifications

  • Required:
    • New York State LMSW, LCSW, or LMHC and a master’s degree in social work, counseling, or related field.
    • Minimum 3 years of post-master experience providing clinical services in behavioral health or substance use settings.
    • Demonstrated use of evidence-based treatment modalities (e.g., CBT, MI, DBT, and trauma-informed approaches).
    • Strong documentation and organizational skills; proficiency with Electronic Health Records (EHR).
    • Commitment to equity, inclusion, and culturally responsive
  • Preferred:
    • System-oriented and interested in community work
    • Bilingual English/Spanish
    • Familiarity with AWARDS or comparable EHR
    • Experience collaborating with peers, case managers, or interdisciplinary
Responsibilities
  • Provide individual, group, and family psychotherapy using evidence-based and recovery-oriented practices.
  • Conduct intake assessments, treatment planning, and progress documentation in compliance with New York State regulations and agency policies.
  • Deliver integrated treatment to clients with co-occurring mental health and substance use
  • Engage clients through community-based outreach, including home visits and off-site
  • Collaborate closely with peers, housing and employment specialists, and other members of the interdisciplinary team.
  • Utilize collaborative documentation during sessions to promote transparency and client
  • Support the ongoing development of integrated clinical services within the Article 31
  • Participate in staff meetings, clinical supervision, and case
  • Contribute to program initiatives that strengthen access, engagement, and client
  • Perform other duties aligned with CONNECT’s mission and evolving program
Essential Physical Job Functions
  • Ability to walk throughout the Lower East Side, including stairs and sidewalks, to engage clients in community settings.
  • Ability to carry materials such as folders, a tablet, and a cell phone (up to 20 pounds) for several hours per day.
