Hamilton-Madison House (HMH) is the leading provider of behavioral health services for New York City's Asian community and the second largest provider in the country. HMH serves nearly 1,000 community members through a comprehensive program model that understands and incorporates the strengths of individual cultures and addresses the stigma related to mental illness. Services range from psychological services; personalized recovery-oriented services for individuals with mental illness; recovery programming for addiction in six languages and seven dialects; supportive housing for people with mental health issues; and training and internship opportunities for graduate students in social work, mental health counseling, and psychology. Our team of over 40 full-time and 16 part-time staff speak Cantonese, Mandarin, Korean, Japanese, Vietnamese, and Cambodian.
HMH is hiring for a full-time Behavioral Health Clinician responsible for direct clinical work with clients in the outpatient mental health clinic, with a priority focus on those in the CONNECT program. The BH Clinician will also be responsible for coordinating and accessing care for their clients. The CONNECT program is a Department of Health and Mental Hygiene (DOHMH) Demonstration Project to provide rapid access to mental health services in a more flexible and holistic way with the community and the whole person outside the clinic walls and beyond the traditional doctor/therapist-patient interaction. The position will develop community linkages to address immediate concrete needs to improve the quality of life of the people being served. The position requires community outreach and engagement, providing psychiatric counseling and social services to individuals, families, and groups affected by mental health or co-occurring disorders. The BH Clinician reports to the CONNECT Program Director. This is a clinic and field-based position, traveling to various sites in Lower Manhattan is required.
Responsibilities:
- Conduct intake assessments, evaluate symptoms and treatment course, crisis intervention, and discharge planning
- Provide individual and group psychotherapy using different modalities
- Utilization of treatment services such as individual, group, case management, comprehensive psychosocial assessments
- Assessments will also include housing needs, vocational/educational needs, and other concrete needs as it relates to social determinants of health and develop objectives and recommendations for treatment services
- Coordinate client care by scheduling and accompanying to medical, legal and other necessary appointments and completion of housing and social services applications
- Ability to collaborate and coordinate care with a multidisciplinary and diverse treatment team
- Work effectively with community providers, education and vocational training providers, legal and self-help groups providing services to CONNECT clients.
- Ability to work with an underserved and diverse group of individuals, including substance use and/or justice-involved, etc.
- Provide after-hours crisis intervention on-call services on a rotating basis
- Maintain medical records documentation according to HMH, OMH, and/ OASAS policies and procedures
- Provide education and teaching to clients and non-clinical staff on evidence-based treatments as needed
- Attend case conferences, program and division wide meetings as necessary
- Complete and keep up-to-date accurate clinical records such as comprehensive evaluation, treatment plan, progress notes and reports regarding the patients' histories and progress, services provided and other required information
- Provide patients or family members with information with mental health issues and about available services and programs and make referrals when necessary
- Meet productivity and quality assurance goals as set forth by the department & agency
- Other duties as assigned as necessary to support clinic operations by supervisor or Assistant Executive Director of Behavioral Health
- Master's Degree or limited permit (LMHC, MHC-LP, LMSW/LCSW)
- Experience in substance abuse and mental health treatment services preferred
- Bilingual English/Asian language a plus
- Good oral & written clinical, communication, and interpersonal skills
Compensation: $55,000 - 58,500
Excellent benefits including medical, dental, and vision insurance, 15 vacation days, 12 sick days, 3 personal days, and 12 paid agency-wide holidays.
Hours: Monday to Friday, 37.5 hours per week, with some weekends required
