Responsibilities

The Clinical Social Work Specialist is an essential member of the care team, playing a vital role in coordinating care for patients facing multiple social stressors and/or behavioral health issues. This position involves utilizing screening criteria to enhance care quality, reduce service fragmentation, and improve patients' quality of life.

Essential Duties and Responsibilities

• Manage a caseload of high-risk, socially complex patients
• Conduct face-to-face and telephonic psychosocial assessments with patients and/or caregivers
• Identify psychological and social determinative barriers and design effective interventions to address care gaps
• Provide short-term counseling services as necessary
• Coordinate services ensuring patients and families understand treatment plans
• Collaborate with a care team to achieve optimal quality and patient experience outcomes, actively participating in High-Risk Huddles
• Outreach to community services to support patients in aging at home
• Develop and implement shared goals of care with patients and families
• Establish relationships with community agencies to provide resources supporting patient independence

Qualifications

Required Knowledge, Skills, and Experience

• Master's degree in Social Work
• Current licensure as an LCSW or LISW (depending on state requirements)
• Valid driver's license and a good driving record
• Strong clinical judgment and clear communication skills in both written and verbal formats
• Proficient computer skills
• Ability to be self-directed and effectively communicate across multidisciplinary teams
• Exceptional time management skills, able to balance multiple tasks, meet critical deadlines, analyze trends, identify system improvements, and implement changes
• Capability to share expertise and understand the necessity of performance improvement while maintaining patient satisfaction and operational efficiency

Preferred Knowledge, Skills, and Experience

• Experience in Care Management within community settings or equivalent
• Experience working with chronic, complex, and/or behavioral health populations
