As a Case Manager, you'll play a pivotal role in supporting our clients from admission through discharge, guiding them through their treatment journey and ensuring they have the tools and support needed to thrive.

What You'll Do:
- Serve as the primary point of contact for clients during treatment, ensuring continuity of care from intake through discharge.
- Develop personalized treatment plans, identify key problem areas, and collaborate with clinical staff to ensure goals are measurable and achievable.
- Facilitate individual and group sessions using Evidence-Based Practices.
- Coordinate aftercare plans and make referrals to support long-term recovery.
- Manage a caseload of up to 12 clients with regular documentation and treatment planning sessions.
- Intervene in crisis situations and manage client transitions across levels of care.
- Maintain active participation in clinical team meetings, quality assurance efforts, and ongoing training.
- A Bachelor's degree in a behavioral health or related field (Master's preferred).
- Clear background screening and compliance with federal, state, and company policies.
- Valid driver's license with insurable driving history.
- Current CPR/First Aid certification and TB clearance.
- Excellent written and verbal communication skills.
- Knowledge of clinical and medical terminology related to behavioral health.
- Proficiency with Microsoft Office, Adobe, and G Suite applications.
- You're organized, self-directed, and capable of managing complex treatment plans.
- You excel at building rapport with clients, families, and multidisciplinary teams.
- You approach every situation with empathy, professionalism, and accountability.
- You understand the importance of clinical ethics and uphold the highest standards of care.
