Position Summary

We are seeking a dedicated and compassionate Hospice Medical Social Worker (MSW) to join our care team. As an MSW at Haven Hospice, you will play a key role in supporting patients and their families through psychosocial assessments, counseling, and resource coordination. You will help address emotional, financial, and community support needs, ensuring quality of life and dignity during end-of-life care.

Responsibilities:

  • Conduct comprehensive psychosocial assessments of hospice patients and their families
  • Develop individualized care plans in coordination with the interdisciplinary team
  • Provide counseling, emotional support, and crisis intervention to patients and families
  • Assist with advance directives, end-of-life planning, and community resource referrals
  • Participate in interdisciplinary team meetings and care conferences
  • Advocate for patient rights and ethical decision-making
  • Maintain accurate and timely clinical documentation in accordance with state, federal, and organizational guidelines
Qualifications:
  • Master's degree in Social Work (MSW) from an accredited institution (required)
  • Active Licensed Medical Social Worker is REQUIRED in the State of Arizona (required)**
  • Minimum 1 year of experience in a healthcare, hospice, or palliative care setting (preferred)
  • Strong communication, organizational, and interpersonal skills
  • Ability to work independently and collaboratively within a multidisciplinary team
  • Valid driver's license and reliable transportation (home visits may be required)
